By Mr. Marco Scarci, Consultant Thoracic Surgeon

Not every patient I meet ends up having surgery (patients decide for many valid reasons). And that’s exactly how it should be.
It may sound strange coming from a surgeon, but I believe that one of my key responsibilities is to help patients make the right decision for them — even if that means choosing not to proceed.
Here are some of the most common reasons patients decide against surgery, and why I fully respect (and often support) their choice:
1. The symptoms are manageable
Sometimes patients are living with discomfort, but it isn’t limiting their life in a major way. When we weigh the risks and benefits, they may conclude that it’s better to monitor the condition for now rather than move forward with surgery. This is a perfectly reasonable and often wise decision.
2. The timing isn’t right
Life doesn’t pause for a procedure. Patients may be caring for a family member, in the middle of a career transition, or simply not mentally ready. Choosing to wait until the timing feels right can be an important part of achieving a successful outcome.
3. They want to try conservative management first
Some conditions can be managed (at least for a time) with medication, lifestyle changes, or physiotherapy. Exploring non-surgical options first is completely valid—and in many cases, it’s what I recommend.
4. Fear or anxiety
It’s natural to feel nervous about surgery. For some, that fear becomes overwhelming. Rather than dismissing these feelings, I take them seriously. We talk through the fears, the facts, and sometimes the conclusion is: not now, or not ever. That’s OK.
5. They want a second opinion
Some patients want to hear from another specialist before deciding. I welcome this. A good second opinion can provide peace of mind, clarity, or even a different set of options. Your health decision should be based on trust, not pressure.
The Bigger Picture: Why Patients Decide
Surgery is never the goal. Better quality of life is.
If we talk through your situation and you decide not to proceed, that doesn’t mean the consultation was a waste. On the contrary, it means we’ve done our job: we’ve given you the information, space, and support to make the decision that’s right for you.
And if your situation changes in the future? You’re always welcome back.
