An honest word about radical surgery.
If you have searched for mesothelioma treatment, you will have found centres — often overseas — offering major operations to remove the lining of the chest, sometimes with heated chemotherapy. Desperation makes those pages persuasive. You deserve to know what the evidence actually shows.

Asbestos, decades ago — symptoms, now
The exposure
Almost all mesothelioma is caused by asbestos, typically breathed in 20–50 years before diagnosis. Construction, shipbuilding, plumbing, electrical work, carpentry, lagging and demolition are the classic trades — and family members exposed through work clothes can be affected too.
The symptoms
Increasing breathlessness (usually from fluid), chest wall ache, fatigue, unintended weight loss, or a persistent cough. Symptoms are often gradual — which is why they are so easily attributed to age or fitness for months.
When to act
Fluid around the lung in anyone with possible asbestos exposure needs proper investigation — not just repeated draining. If that is your situation, or a scan has already raised the question, it warrants a specialist review now.

Mesothelioma questions, answered honestly
Fluid drawn from around the lung frequently fails to confirm mesothelioma — the cells it contains are often too few or too ambiguous, which is why patients can have several “inconclusive” drains. The definitive test is a tissue biopsy of the pleura itself, and a VATS keyhole biopsy under direct camera vision provides generous, well-targeted samples with a very high diagnostic accuracy — enough tissue for the subtype and biomarker testing that modern treatment depends on.
A procedure that seals the space where fluid collects. During the VATS operation the fluid is fully drained and sterile medical talc is applied to the lung lining, causing the two surfaces to adhere so fluid can no longer accumulate between them. When the lung can re-expand fully, it is highly effective at stopping the fluid for good — and it is done in the same anaesthetic as the biopsy.
A soft, discreet tube placed under the skin that allows fluid to be drained at home, in minutes, by you, a family member or a district nurse — no more emergency admissions for drainage. It is the right choice when the lung is “trapped” by disease and cannot re-expand enough for pleurodesis to work. Which approach suits you is decided on your scans and at the time of surgery.
You deserve a straight answer: the best evidence — from the UK-led MARS 2 randomised trial — showed that radical surgery did not extend life and left patients with more complications and worse quality of life, which is why it is not recommended in UK practice and why I do not offer it.
